Sports Physicals and Medical Forms
New York State Public High School Athletic Association requires all student-athletes to have a physical exam in order to participate. Athletes will not be allowed to participate in any practice or game until they have a NYS physical exam completed and on file in the Health Office.
The NYS School Health Examination form and the Parent Permission to Treat form are both available and must be submitted through August Schools. Physical exams for day students will not be provided by Ross.
The physical exam must be conducted and signed by a certified physician, on file in the Ross School Health Office, and cannot be older than 12 months at the start of the season of play.
Contact your family doctor and arrange for a sports physical exam prior to arriving on campus for the beginning of the school year.
